You can pay for the fitness assessment using the Scan to Pay function on the Momentum App.
Log in to the Momentum App, and select Momentum Medical Scheme;
• Tap on Scan to Pay with HealthSaver;
• The camera on your device will open and you will be able to scan the QR code; and
• The amount will appear on your device for you to approve, or you will be prompted to enter the
amount and approve the transaction.
Alternatively, you can use your bank account and claim back the amount from your HealthSaver account. You can forward your invoice and proof of payment to claims@momentumhealth.co.za, and the claim will be processed. Momentum Medical Scheme will then refund you from your HealthSaver, provided there are sufficient funds in the account.
Please check with the health professional when you make the booking, as they have various accepted payment methods.
