1. Purpose
The purpose of these terms and conditions is to give you information and guidelines on how the new wellness rewards products, Multiply Inspire and Inspire Plus and Multiply Engage and Engage Plus, will work.
2. Parties and/or participants
2.1. Momentum Multiply – refers to Momentum Multiply Proprietary Limited herein referred to as “We” or “Us” or “Our” or “Momentum Multiply”.
2.2. Member – refers to main member; partner/spouse; adult dependant; and child dependant herein referred to us as “you” or “your”.
3. Acceptance of these terms and conditions
3.1. By participating in any Momentum Multiply assessment, you agree to have read, understood and accepted these rules. You also agree to the Momentum Multiply General Programme Rules and all the different sets of terms and conditions related to digital channel usage, privacy policy, status and tiers, partner terms and conditions (channel and product).
3.2. These rules will apply to and bind all Momentum Multiply member(s). Dependants on your membership are also bound by these general rules. Any rule that applies to the main member, will therefore also apply to dependants.
4. Health assessment general information
4.1. You can go for a health assessment at a Momentum Multiply affiliated health professional.
4.2. This assessment will measure certain health markers to give you a Healthy Heart Score.
4.3. Your Healthy Heart Score will determine the level of HealthReturns that you will receive (Multiply Inspire and Inspire Plus members).
4.4. The partner rewards level for you as a Multiply Inspire Plus and Multiply Engage Plus member will be determined based on your fitness assessment result and your Healthy Heart Score.
5. Eligibility
5.1. This assessment is available to all main members, their spouses or partners and adult dependants on Momentum Multiply Inspire, Inspire Plus, Engage and Engage Plus, 18 years and older.
6. Validity
6.1. The results of the health assessment (Healthy Heart Score or HHS) are valid for 12 months.
6.2. The validity of the HHS will be influenced by change or expiry of any of the input measurements.
6.3. The following set of measurements have a 12-month validity period. Once one of them has expired, the HHS also expires until the expired measurement is updated again:
6.3.1. Blood pressure
6.3.2. Glucose
6.3.3. Total cholesterol
6.3.4. Body fat percentage
6.3.5. Smoker status
7. Calculation of your Healthy Heart Score (HHS)
7.1. A set of components and measurements are required to calculate your HHS. These include blood pressure, glucose or HbA1c (optional), total cholesterol, HDL cholesterol (optional), body fat percentage together with smoker’s status.
7.2. The calculations of all the measurements are added together to get a final score, which will place you in one of the following categories:
7.2.1. Red: You need to take strong action to reduce your cardiovascular disease risk factors.
7.2.2. Amber-Red: You should take action to reduce your cardiovascular disease risk factors.
7.2.3. Amber: You should take action to reduce your cardiovascular disease risk factors.
7.2.4. Amber-Green: You should take action to reduce your cardiovascular disease risk factors.
7.2.5. Green: All your cardiovascular disease risk factors are under control for now.
7.3. You will receive HealthReturns based on your Health Heart Score. After the assessment, you’ll receive comprehensive feedback on each component of the test.
7.4. An HDL and HbA1c readings are optional – you can do these tests and pay for them yourself. Momentum Multiply will accept the results, if not older than 6 months and these apply to the HHS.
8. Rewards based on Healthy Heart Score
8.1. You earn HealthReturns based on the result of your health assessment (your Healthy Heart Score).
8.2. HealthReturns are paid for 12 months or as long as the results of the health assessment are valid.
8.3. HealthReturns will be paid into your HealthSaver account, so you need an active HealthSaver account.
9. Booking a health assessment
9.1. You can do your health assessment at any Dis-Chem, Clicks, Pick n Pay pharmacy, Momentum Multiply approved independent pharmacies and health professionals registered with Momentum Multiply.
10. Fees and charges
10.1. If you earn HealthReturns you will pay R40 per year administration fee for your HealthSaver account.
11. Apps
11.1. All engagement for Momentum Multiply Inspire and Inspire Plus and Momentum Multiply Engage and Engage Plus will take place on the Momentum app or the Momentum Multiply app.
11.2. Momentum clients will have access to Momentum Multiply and the relevant products through the Momentum app and all other members will have access through the Momentum Multiply app.

11.5. Once you’ve downloaded the respective app, you need to register a profile. You will be able to view all your health and membership details on your private profile.
11.6. Please note the Momentum Multiply app terms and conditions will apply, you can find these on the app.
12. Termination and cancellation
12.1. You will no longer have access to this benefit if your Momentum Multiply membership is cancelled.
12.2. On termination date of the main member’s Momentum Multiply membership, all members rights to the benefits will cease and no refunds of any fees paid will be permitted. Refer to the Momentum Multiply General Programme Rules for details.
12.3. Momentum Multiply may cancel or suspend your benefit upon investigation proving one of the below:
12.3.1. Your breach any of the rules of Momentum Multiply; or
12.3.2. You are using the benefit(s) of the program in an abusive and/or fraudulent manner.
12.4. Momentum Multiply may cancel your benefit upon conclusion investigation proving one of the abovementioned breaches.
13. Change to the terms, conditions and rules
13.1. Momentum Multiply may, in its sole discretion and at any time, amend these rules and/or any business rules associated with them in any manner whatsoever. Such amendments may include, but shall not be limited to, amendments to the amounts and percentages referred to in these rules as well as changing the model of the Momentum Multiply programme.
13.2. Momentum Multiply shall publish the amended terms and conditions on Momentum Multiply digital channels or any other manner as Momentum Multiply may determine in its sole discretion.
13.3. You will be provided with thirty days prior notice to any amendments and if you continue to use the Momentum Multiply benefits at the end of the notice period (effective date), you will be deemed to have accepted the amendments.
14. Important disclosures and legal matters
14.1. If you fail to comply with any part of these rules, you will be disqualified and you will forfeit any benefit(s).
14.2. Should we not exercise some or all of our rights or waive certain requirements, this shall not be seen as a waiver of our rights or deprive us from insisting on strict adherence to those terms or any other term at a later stage. You must not assume that this means that the rules have been changed or that it no longer applies to you.
14.3. Momentum Multiply is not responsible/liable for any unauthorised human or technical intervention, or for any transactions that is not processed, executed, saved or transmitted, regardless of the cause.
14.4. Momentum Multiply may at its sole discretion decide who may participate in these benefits/challenges.
14.5. Any benefit allocated to you considers prevailing legislation and/or any applicable practice. In the event of a change in the applicable legislation and/or practice, Momentum Multiply reserves the right to adjust the benefits accordingly without any reasonable notice to you.
14.6. If one or more of these rules is invalid, unlawful or unenforceable, such portions will be severable from the body and it will not impact on the validity of remaining rules, which will stay valid.
14.7. You may not transfer or cede or exchange or sell membership, benefit or reward rights to another person.
15. Personal information and privacy
15.1 Momentum Multiply will handle your right to privacy according to the rules and regulations as stipulated here.
15.2 You will be asked to agree to certain privacy settings when registering a profile on the Momentum or Momentum Multiply app. These privacy settings can be updated at any time
15.3 Your assessment results will be kept confidential and will only be shared with the relevant third parties with your consent. We will not share your results with other members. This will only be visible on your dashboard after you’ve logged in to the app.

17. Definitions
17.1. Adult dependant means dependants 18 years and older.
17.2. Child dependant means persons aged 17 years and below.
17.3. Momentum Multiply General Programme Rules shall mean the main terms, conditions and rules for the Momentum Multiply Program which will apply to all Momentum Multiply users who utilise and accept these rules.
17.4. HDL – High-density lipoprotein cholesterol.
17.5. HbA1c – A haemoglobin A1C test is a blood test that shows your average glucose level over the past two to three months.
18. Interpretation
18.1. In these rules, the singular always refers to plural, as well as the other way around. In these rules references to the masculine gender shall include the feminine.
18.2. The headings in this agreement will not affect the interpretation of it.
